Hamstring injuries are one of the most common muscle injuries to occur in sport and exercise. They most commonly occur from a sprint or over stretching. Hamstring injuries can range from a small pull or strain, which is often described as a tightness, to a complete tear/rupture which can signficantly limit strength, mobility, and function.

At Optimise Sports Physiotherapy & Performance, we understand the frustrations of hamstring injuries keeping you from doing what you love. Hamstring injuries have a high recurrence rate, therefore they need a good strategies rehabilitation approach to mitigate the risk of re injury. On initial assessment, our physiotherapists will confirm diagnosis, then structure an individualised rehab plan to reduce pain, improve strength, mobility, and power, and have you returning to your sport as quickly as safely possible.

Upper Hamstring Injury

Types of Hamstring Injuries

Common hamstring injuries we see in practice include:

  • Hamstring strains & Tears – ranging from Grade 0 (mild tweak) to Grade IV (complete rupture or tear).
  • Proximal hamstring tendinopathy – tendinitis of the hamstring tendons that attach into the gluteal region (known as the sitting bone)
  • Avulsion injuries – These occur when the tendon pulls of the bone. Depending on the severity, a surgical opinion may be required
  • Delayed onset muscle soreness (DOMS) – This is more related to muscle fatigue/soreness after overloading the hamstrings or doing a new activity. This may increase risk of a strain/tear if not managed appropriately
  • Neural Hamstring Pain – This can often be nerve related pain stemming from the low back masking a hamstring injury
  • Post-surgical Hamstring rehabilitation – Long-term rehabilitation following surgical repair of the hamstring usually involving the tendon

Signs & Symptoms of Hamstring Injuries

Common signs & symptoms include:

  • Sudden sharp pain, pull, or tightness felt at back of the thigh, with some describing it as a really bad ‘cramping’ feeling
  • Feeling a pop (more often with higher grade injuries)
  • Swelling and bruising can be seen in more serious muscle/tendon hamstring injuries
  • Difficulty weigh bearing (e.g. walking, sitting down, going up and down stairs) due to pain, loss of strength and reduced mobility

If you experience of these, seeing a physiotherapist is important to get an accurate diagnosis so you can manage your injury appropriately without doing further damage.

Why Personalised Treatment is Essential

No hamstring injury is the same, with numerous factors including your training age, injury history, the type of strain/tear you have, and your running technique to name a few all influencing how treatment is done.

At Optimise Sports Physiotherapy & Performance, we provide:

  • Detailed assessment of hamstring strength, mobility, and movement mechanics
  • Individualised Exercise programs to help you recover from your injury and get back to pre injury performance, if not better
  • Manual therapy (ie sports massage and dry needling) to reduce symptoms of pain and stiffness as part of a comprehensive rehabilitation plan
  • Return to Sport Guidance with advice and progressions given depending on your sport and exercise to have you return as safely as possible

Recovery Timeline

Grade Expected Recovery Details
Grade 0 (tweak/soreness) 0-1 weeks Typically just soreness or fatigue. No strain has occurred
Grade 1 (mild strain) 2-3 weeks Mild strain, can recover quickly with early rehab
Grade II (Moderate strain) 3-8 weeks Determined by function and location of tear
Grade III (High Grade Tear) 2-6 months More time is needed for full tissue healing
Grade IV (Rupture) 5-6+ months Surgical intervention may be required

Why Early Physiotherapy Matters

Early treatment is important to have an accurate diagnose and treatment plan catered towards your needs to begin your recovery as quickly as possible. Other benefits include:

  • Treatment that can help reduce pain (light massage, Normatec compression boots, cryotherapy)
  • Minimise stiffness and loss of mobility
  • Early safe strengthening exercises to build tissue capacity quicker
  • Establishing a plan that builds confidence in returning to sport safely

Do you have pain in or around your hamstring, or do you have a suspected injury in this location? Early assessment and treatment can make a big difference to your recovery. At Optimise Sports Physiotherapy & Performance, we’ll accurately diagnose you with our comprehensive assessment, and start the rehabilitation and treatment process straight away to get you back to your goals at full function, pain-free.
